Flexible, hourly attendant care and skills training for adults with intellectual and developmental disabilities — in their home or in the community.

What Are Community Living Supports?

Community Living Supports (CLS) are flexible, hourly services delivered to adults with intellectual and developmental disabilities in their own home or in the community. Unlike residential programs, CLS does not involve the individual living in a provider-owned or operated setting. Vitalia comes to where the individual lives or supports them in the community — based entirely on what their ISP authorizes.

Services Include

Attendant Care

Hands-on assistance with activities of daily living (ADLs) and instrumental activities of daily living (IADLs).

Skills Training

Structured support to build or maintain independent living skills — based on the individual’s ISP goals.

Relief Care

Temporary support to give primary caregivers a scheduled break, with continuity of care for the individual.

Community Access

Support for participation in community activities, errands, social connections, and engagement outside the home.

Who Is This For

Adults (18 and older) with intellectual or developmental disabilities who:

  • Live in their own home, a family home, or another non-provider-owned setting
  • Are eligible for Oregon ODDS developmental disabilities services
  • Receive Medicaid through OSIPM or Healthier Oregon
  • Have an ISP that authorizes Community Living Supports hours

How CLS Works at Vitalia

Vitalia operates as a Standard Model Agency under Oregon rules. This means:

We employ, train, and supervise all Direct Support Professionals directly
We develop an individualized Implementation Strategy for each person in coordination with their case manager
We submit quarterly progress reports to the case management entity
Each individual has a dedicated program supervisor accessible during service hours
We do not require individuals or families to coordinate DSP schedules themselves

Our Team

Vitalia staff speak English, Spanish, Amharic, and Swahili. We actively recruit bilingual staff and engage translation services to ensure individuals receive supports in their primary language. Language access is built into our staffing model — not an afterthought.

Regulatory Authority

Vitalia Care Services is certified and endorsed by the Oregon Department of Human Services, Office of Developmental Disabilities Services (ODDS) to deliver Community Living Supports as a Standard Model Agency under Oregon rules.
